Well by this I mean, what is the biggest plot twist you have seen as in an unexpected devolpment in a game.
For me it had to be that (SPOILERS)
Luigi is the final boss in Super Paper Mario and Dimentio is the main villan instead of Count Bleck. Also Dimentio has been “helping” you the entire game.

In Pokemon Mystery Dungeon: Explorers of Sky, at the end of Episode 5, when Dusknoir decides to change his ways and helps Grovyle, and Celebi save the future.
Not really a plot twist but still… After playing Crash Bandicoot 3, then playing Crash Bash and seeing “ZOMG Dingodile and Tiny are on the good side!? BLASTPHEMY”
You say real whereas I much prefer to hold defeating Primal Dialga as the true ending.
Defeating Darkrai meant nothing to me except “Yaay I can evolve now!”
Whereas, when you defeated Primal dialga, not only where you at the top of a crumbling tower but you have the epic theme song of Primal dialga in the background. Not only that, but, you had saved all of ime itself, saving everyone you held near and dear, despite the fact that you knew yourself that you would have to disappear.
Now that might just be me rambling but that was what I understood you to be implying.

Eh? I wasn’t aware you fought Voldemort at the end of PMD2…
But the point is I don’t like the “Real” ending as you so put it, because it just seemed to be feeding off the previous story, because lets be honest it all links back to the collapse of Temporal Tower. Sure Darkrai caused it but when you fought him he was flanked by pathetic pokemon that could easily be taken out with your eyes closed. Now comes to my other main problem with it. Cressilia has to be the absolute worst escort, because she isn’t an escort she is supposed to be the one making it all easy for you! Ridiculous! You are saving her behind at every battle! It shouldn’t be like that. Personally I blame the terrible AI, but we never had that problem against Primal Dialga, as it was just you and your partner in a fight to the finish.

Fight against Kefka, thinking to yourself how that evil could never truly triumph and that he could never set the world into chaos, but he actually did cause it to happen, splitting up your team and, if you hadn"t waited, killed one of your team mates (Oh Ninja, I lurved you soooo ;.; )
Let’s see… There aren’t many plot twists that I’m aware of. I mean, I expect the unexpected. Biggest… Hm… Well, I don’t know really. I’ll go with Dusknoir’s story in PMD2, though I thought of him as evil in the first place.
Hmm… When Tohru Adachi is revealed as the one of the main antagonists in Persona 4.
Then when Shuji Itkutsuki is also revealed to be evil and kills Mitsuru Kirijo’s father in Persona 3. I was one of the only people who actually found his jokes funny. :l

The next one is a strange case. I don’t really think it’s a plot twist, and it’s CRAZY HARD to describe. But I’ll explain it anyways.

…well, basically she turns out to have a pure white face, green hair, an unaturally big grin, and she dances, all the while circles swirl around her, with penguins and other animals just walking around the swirls.
My biggest suprise is from pmd3 Gates to Infinty (SPOILER)
when munna calls for help and turns you into a pokemon the when you rescue her she and many pokemon attack and try to kill you to prevent the world from being destroyed. she mentions that hydreigon is ruining the world but is actually the one who brought you to the world and saves you. hydreigon is the voices of life in reality. munna works for kyurem to help protect the bittercold from being destroyed so the world is destroyed. the turn of munna was most suprising to me to sum things up because she appears in your dreams in many occations to tell you to save her and the world
I feel sorry for you, then. Persona 3 is one of my favorite games ever, and from what I’ve heard, Persona 4 improves on it pretty well.
I can’t list just one here, so I’ll name a few.

[spoilerBatman: Arkham City:y06t41qt]When you find out that the healthy Joker you"ve been seeing for most of the game is actually Clayface, and the real Joker is still deathly ill. When I beat this game, it was probably the worst I"ve ever felt at the death of a villain. This bit kicks off the events that lead up to the Joker"s death scene, which just gives it even more of an impact.
